Plaintiff’s proceedings supplemental in garnishment having come on for hearing and the finding that plaintiff is the owner of unpaid judgment, costs, and interests against said judgment defendant, person you filed against, and that there is unpaid upon said judgment and costs the sum of $total amount of judgment & court costs and that it appears to the satisfaction of the Court that proof of service of notice of hearing has been had on the defendant and on the garnishee defendant, and that the plaintiff is entitled to a final order in garnishment for the collection thereof.

IT IS, THEREFORE, ORDERED, ADJUDGED, AND DECREED BY THE COURT that the garnishee defendant, employer name, deduct and withhold from the wages, salaries, earnings and income of the judgment defendant, person you filed against, the lesser of the following sums:

(1)

Twenty-five percent (25%) of the judgment defendant’s disposable** earnings for each workweek; or

(2)

The amount by which the judgment defendant’s disposable earnings for each workweek exceed thirty (30) times the federal minimum hourly wage prescribed by Section 6(a)(1) of the Fair Labor Standards Act of 1938, U.S.C.; Title 29, Section 206(a)(1), in effect at the time earnings are payable.

That the garnishee defendant shall pay said deductions to the Clerk of this Court until the above judgment, costs, and interest shall be fully paid and satisfied.
