As the parent or guardian of the user(s) named in the agreement, I understand and agree that I am responsible for my child’s internet and computer activities, whether I am present or not. I also do not hold the library responsible for content accessed, intentionally or unintentionally, by my child or other patrons. I have read, understand, and agree to the regulations and consequences outlined in the Duchesne County Library System’s Internet and Computer Access Policy and Procedures. I hereby certify I am the legal parent or guardian of this child and give my permission to use the library’s internet and computer services.
