A land use application assists the Community Development Department administer land use services. Applicants use this form to convey the intent of their proposed project to Department staff. A complete application includes this form, any applicable fees, and may include additional documents, such as the following: surveys, site maps, site photos, project narrative, drawings, title reports, various permits, licenses, and supplements.

Please fill out this form in its entirety. Submit the completed form and all applicable attachments to the Duchesne County Community Development Department for an application review. If deemed complete by the Department, the date for a Planning Commission decision will be assigned. Please note: a complete land use application does not equate to a land use decision, approval, or denial.
