Statement of Concern

Duchesne County has delegated the responsibility for selection and evaluation of library resources, events, exhibits, etc. of the Duchesne and Roosevelt branches to the Duchesne County Library System Board of Trustees and the library staff. Procedures have been established to address resident patron concerns about those resources, events, exhibits, etc. To officially express such concerns, please fully complete this form and deliver it to the library in person or electronically for the Library Director’s review.
