Proposal for Deficiency

We are pleased to quote you the following, subject to the terms and conditions:

Unless otherwise stated in this proposal, all work will be performed during regular Koorsen Fire & Security business hours.

Koorsen Fire And Security Technicians will perform and/or provide the following based on results of the most recent inspection(s):

Provide, Replace and Dispose of:

• (2) 12v 33ah Batteries that failed inside the fire alarm panel.
• (1) duct detector (RTU 1) that did not function properly.

In addition:

• Koorsen Fire & Security Technician(s) will assess (1) trouble (DLC @ Address 2 Ground Fault) that were reported on the main fire alarm control unit (panel) upon arrival and communicate with customer next steps based on findings.

Koorsen Fire & Security Technician(s) will function test all repaired and/or replaced devices to ensure proper operations following service(s) and before placing equipment/system(s) back in-service.

• Price does not include any additional work other than what has been outlined in the scope of work above. Any additional approved work will be billed at time and material.
• Price does not include unknown associated fees as they may apply.
• This proposal does not include fire watch as required by NFPA 101 Standard. Customer shall contact AHJ.

* A convenience fee of 3% of the invoice amount will be charged for payments made by credit card.

* Please be advised that due to the evolving international trade environment, pricing is subject to change if tariffs are implemented prior to proposal approval or material procurement.

This proposal is valid for 30 days.
