This Addendum to the Lease Agreement (“Addendum”) is entered into by and between the Delaware County Board of Commissioners (“Landlord”) and Muncie OpCo, LLC (“Tenant”) on this 16 day of February, 2026.

WHEREAS, Tenant will operate a County-owned Addiction Recovery Center. Tenant has assessed the inside of the facility and is requesting permission to make alterations in order to better serve the patients of the Recovery Center pursuant to Section 6.05 of the Lease Agreement; and

WHEREAS, the Landlord has reviewed the Tenant’s request for facility modifications. The lease permits facility improvements and alterations with County approval, and the proposed changes have been evaluated for consistency with the intended use, safety, and ongoing operation of the facility; and

WHEREAS, the Landlord is in agreement that the requested modifications are in the best interest of serving the intended public.

IT IS THEREFORE AGREED BY THE TENANT AND LANDLORD:

1. Tenant, Muncie OpCo, LLC has provided a scope of work outlining the following modifications:
   1. Exterior Improvements
      1. Installation of gazebos, a fire pit, planter boxes, and outdoor furniture in the backyard area; and
      2. Construction of concrete pads to support the gazebos; and
      3. Installation of a six-foot black fence enclosing the backyard area.
   2. Interior Improvements
      1. Replacement and modification of door hardware; and
      2. Application of privacy film on doors looking into resident rooms; and
      3. Installation of locking mechanisms on thermostats; and
      4. Installation of baby changing tables; and
      5. Conversion of an existing locker room into a staff board room; and
      6. Replacement of the security window in the lobby; and
      7. Reprogramming of the access control system to operate in a fail-safe mode.
