Environmental Health Specialist in Training (EHSIT) or Registered Environmental Health Specialist (REHS) on the second re-inspection.

Failure to correct situations of public health significance, when numerous or specific types of critical or non-critical violations are documented, or when repeat or failure to maintain violations are documented on a second re-inspection at the discretion and determination by the Supervisor will either result in a third re-inspection depending upon circumstances or result in a Pre-Hearing Conference to be scheduled at the discretion of PHDMC Supervision and the PHDMC Senior Supervisor of General Services or the Oakwood City Director of Environmental Health.

During the Pre-Hearing Conference, both parties will arrive at a mutually agreeable timetable for correction of violations and a Consent Order outlining the Agreement is issued.

Failure to satisfactorily correct the items agreed to in the Pre-Hearing Conference will result in a formal Hearing before Oakwood City Health Commissioner or their designee (the Oakwood City Director of Environmental Health) to determine further action required to ensure compliance with the Consent Order previously agreed to which may include but is not limited to; more time to comply with initial orders or mandate an appearance before the Oakwood City Board of Health.

After a Hearing before the Oakwood City Health Commissioner or their designee, if further time has been granted to comply with the Consent Order previously agreed upon, and the Operator has still failed to meet the requirements; said Operator shall then appear before the Oakwood City Board of Health to show cause as to why the operation license shouldn't be suspended or revoked.
