OAKWOOD BOARD OF HEALTH

WHEREAS, on November 18, 2024, pursuant to Section 3717.11 of the Ohio Revised Code, the Ohio Department of Agriculture (ODA) conducted a survey of the Board’s Retail Food Establishment (RFE) program, which is operated through a contractual relationship with Public Health – Dayton & Montgomery County (PHDMC); and

WHEREAS, the ODA determined that the Board’s RFE program is performing in a satisfactory manner, in substantial compliance with applicable provisions of the Ohio Revised Code and Ohio Administrative Code, but nevertheless required the Board to approve and submit an action plan pertaining to three (3) items in the survey;

NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF HEALTH OF THE CITY OF OAKWOOD, MONTGOMERY COUNTY, OHIO, THAT:

Section I.
To ensure ongoing compliance with all applicable laws and administrative regulations, the Oakwood Board of Health hereby approves and adopts the Action Plan set forth on the attached Exhibit A, which is incorporated by this reference.

Section II.
In support of the Action Plan referenced in Section I, the Oakwood Board of Health hereby approves and adopts the Enforcement Procedure policy attached hereto as Exhibit B, and the Training Protocol policy attached hereto as Exhibit C, both of which are incorporated by this reference.

Section III.
This resolution shall take effect upon passage. The Secretary is authorized and directed to transmit a copy hereof to the appropriate office(s) of the Ohio Department of Agriculture.
