Establishment Operations in Oakwood City Health District
The Oakwood City Health District contracts with PHDMC to perform all inspections and administrative support.
At the time of an initial routine inspection, if specific violations (For example: situations of public health significance, when numerous or specific types of critical or non-critical violations are documented, or when repeat or failure to maintain violations are documented) are observed, not corrected in place, and subsequently documented on the inspection form, applying relevant training, knowledge, and experience thus making an informed decision; then orders (with specific time limits) for correction may be issued. If time limits are issued, a required re-inspection to determine compliance with the initial orders shall be conducted.

Re-inspection Flowchart Steps:
- First Re-inspection:
- If Compliance: Resolved (further action not required; routine inspection schedule resumes).
- If Non-Compliance: Escalates to Second Re-inspection.
- Second Re-inspection:
- If Compliance: Resolved.
- If Non-Compliance:
- May advance directly to Pre-Hearing.
- Or advance to Third re-inspection, which subsequently leads to Pre-Hearing.
If compliance is achieved at any level then further action is not required and the next routine inspection will be conducted per schedule.
- In situations of public health significance, when numerous or specific types of critical or non-critical violations are documented, or when repeat or failure to maintain violations are documented they are given corresponding adequate time limits (orders) for correction of each violation or the violations as a whole, unless corrected on site, then a date will be set for a follow up re-inspection to determine compliance.
- Failure to correct situations of public health significance, when numerous or specific types of critical or non-critical violations are documented, or when repeat or failure to maintain violations are documented on an initial re-inspection, will result in a second re-inspection for the outstanding violations and a date is set for said follow up second re-inspection to determine compliance.
